Acute bilateral submaxillitis is a rare event, except when it is caused by sialolithiasis. It has been described secondary to allergic, infectious, suppurative or viral processes, autoimmune such as Sjögren's syndrome, drugs such as thiopurines, nitrofurantoin, phenylbutazone, captopril, and after upper airway procedures such as upper endoscopy, orotracheal intubation bronchoscopy and ERCP (endoscopic retrograde cholangiopancreatography for choledocholithiasis). Treatment with tumour necrosis factor-alpha (TNF-alpha) antagonist drugs is associated with an increased risk of reactivation of intracellular bacterial infections, so that listeriosis has been described in pathologies that require such treatment, such as rheumatic, dermatological and intestinal diseases, which present other comorbidities or are immunocompromised. Listeriosis mainly causes bacteremia and meningitis, when symptomatic, and infects immunosuppressed persons, where it has a lethality despite 30% antibiotherapy. We present the clinical case of a male immunosuppressed patient, secondary to a treatment with azathioprine and prednisone followed by adalimumab, for indeterminate inflammatory bowel disease, superinfected by CMV, who after eating meat contaminated by Listeria monocytogenes, suffered a picture of listeriosis bacteremia, which improved with antibiotic treatment, followed by a transient acute bilateral submaxillitis, which subsided with symptomatic treatment (oral hydration). It is the only case described in the literature, in which an immunosuppressed patient treated with adalimumab, suffers from acute bilateral submaxillitis in the context of listeriosis, caused by the mumps virus.


La submaxilitis aguda bilateral es un evento raro, salvo cuando está causada por sialolitiasis. Se la ha descrito secundaria a procesos alérgicos, infecciosos, supurativos, virales o autoinmunes como el síndrome de Sjögren; a la administración de fármacos como tiopurinas, nitrofurantoina, fenilbutazona, captopril, y tras procedimientos sobre la vía aérea superior, como endoscopia digestiva alta, broncoscopia, intubación orotraqueal y colangiopancreatografía retrógrada endoscópica por coledocolitiasis (CPRE). El tratamiento con fármacos antagonistas del factor de necrosis tumoral alfa (TNF-alfa) se asocia con riesgo acentuado de reactivación de infecciones bacterianas intracelulares, de forma que se ha descrito la listeriosis en afecciones que requieren dicho tratamiento, como enfermedades reumáticas, dermatológicas y del intestino en sujetos que presentaban otras comorbilidades o estaban inmunocomprometidos. La listeriosis provoca bacteriemia y meningitis predominantemente, cuando es sintomática, e infecta a los sujetos inmunodeprimidos, en los que, a pesar de la antibioticoterapia, tiene una letalidad del 30%. Se presenta el caso clínico de un paciente varón, inmunodeprimido, secundario a tratamiento por azatioprina y prednisona seguido de adalimumab, por enfermedad inflamatoria intestinal indeterminada, sobreinfectada por citomegalovirus, que tras la ingesta de carne contaminada por Listeria monocytogenes, sufrió un cuadro de bacteriemia por listeriosis que mejoró con tratamiento con antibióticos, seguido de submaxilitis aguda bilateral transitoria, que cedió con tratamiento sintomático (hidratación oral). Es el único caso descrito en la literatura en el que un paciente inmunodeprimido tratado con adalimumab presenta submaxilitis aguda bilateral en el contexto de la listeriosis, provocada por el virus de la parotiditis.

Resumen Comunicamos el caso de un lactante mayor previamente sano, que luego de tres semanas de recibir la vacuna SPR (sarampión, parotiditis, rubeola) presentó fiebre, aumento de volumen parotídeo y compromiso de conciencia. Se diagnosticó una meningitis aséptica, con pleocitosis en el LCR de predominio mononuclear, detectándose virus parotídeo en LCR por biología molecular. En el Instituto de Salud Pública de Chile se realizó serología (IgM e IgG) que resultó positiva. La muestra de saliva confirmó la etiología por virus parotídeo con genotipo N. La evolución fue favorable, sin secuelas al seguimiento a seis meses. Ante esta situación clínica, se revisó la información respecto a la asociación y causalidad de esta entidad clínica y vacuna SPR, focalizado en diferentes cepas del virus parotiditis.


Abstract We report the case of an older infant with no prior morbidity that approximately 3 weeks after receiving MMR vaccination (measles, mumps, rubella) was hospitalized for feverish symptoms, increased parotid volume and compromised consciousness. Aseptic meningitis was diagnosed, detecting pleocytosis in the CSF, predominantly mononuclear, and confirming by molecular biology, presence of parotid virus in CSF. A study was carried out by the Institute of Public Health of Chile, where serology (IgM and IgG) was positive. Saliva sample confirmed the etiology of parotid virus with genotype N. The evolution was favorable and at 6-month follow-up, there were no sequelae. Given this clinical situation, information regarding the association and causality of this clinical entity and the MMR vaccine, focused on different strains of the mumps virus, was reviewed.

La parotiditis es un infección viral producida por el virus parotídeo. Clínicamente se caracteriza por aumento de volumen de la glándula parótida generalmente bilateral. La estrategia que ha mostrado ser más eficaz para la prevención de esta infección ha sido la implementación de la vacuna tres vírica en los programas de inmunización. En países con población altamente inmunizada como Chile, se logró una importante disminución de la incidencia de esta enfermedad. Sin embargo, a pesar de la efectividad de la vacuna se siguen reportando brotes en todo el mundo, evidenciándose un cambio epidemiológico, trasladándose la edad de presentación clínica desde la niñez y adolescencia hacia los adultos jóvenes. Este aumento en el número de casos ha sido estudiado, determinando que el efecto protector inmunitario de la vacuna decaería con el transcurso del tiempo, contribuyendo a la propagación de los brotes. Con respecto a posibles estrategias para el manejo de los brotes la aplicación de una dosis adicional de la vacunas tres vírica en población expuesta sería una medida que mejoraría el control de los brotes.


Mumps is a viral infection caused by mumps virus. Clinically, it is characterized by increased parotid volume. The most effective strategy for preventing this infection, has been the implementation of measles-mumps-rubella (MMR) vaccine in the national immunization program. Among countries with a highly immunized population, like Chile, there has been an important reduction in the incidence of this disease. Nevertheless, despite the effectivity of the MMR, there are reports of outbreaks worldwide, with an epidemiological change, from clinical presentation in childhood, to adolescents and adults. This outbreaks have been studied, and it has been determined that they are due to the waning of vaccine-derived immunity. Regarding strategies for the management of new outbreaks, the administration of an additional dose of MMR, would be an alternative.

Resumen En Chile, la cepa del virus parotídeo utilizada en la vacuna es Leningrad-Zagreb (L-Z). Aunque la relación entre meningitis y la cepa L-Z sigue siendo controvertida, la mayoría de los casos reportados han presentado un cuadro de curso benigno y sin secuelas neurológicas. Presentamos el caso de una paciente que tres semanas post-inmunización con la vacuna tresvírica evolucionó con una meningitis aséptica de predominio mononuclear, con serología para IgM positiva contra el virus parotídeo. En este caso clínico, existió una relación temporal entre la vacunación, el inicio del cuadro parotídeo y posteriormente el meníngeo; la curva de inmunoglobulinas demostró una infección aguda posterior a la vacuna. Si bien no se logró aislar el virus en LCR, es razonable atribuir el cuadro a una infección post-vacunal.


In Chile, the strain of the mumps virus used in the vaccine is Leningrad-Zagreb (L-Z). Although the relationship between meningitis and the L-Z strain remains controversial, most of the reported cases have shown a benign course without permanent neurological sequelae. We present a case of a patient who presented an aseptic meningitis three weeks after immunization with a mumps vaccine; and laboratory confirmation showed positive serum mumps IgM antibody. In this clinical case, there was a temporal relationship between vaccination and the onset of the mumps and subsequently the meningeal involvement; the immunoglobulin curve demonstrates acute infection after vaccination. Although it was not possible to isolate the virus in CSF, it is reasonable to attribute the picture to a post-vaccinal infection.

Antecedentes. La parotiditis epidémica es una infección viral aguda que produce la inlamación de una o más glándulas salivales, fundamentalmente, la parótida. Es considerada una enfermedad altamente infecto-contagiosa, pero que habitualmente es benigna. con brotes epidémicos que ocurren cada 2, 3 a 7 años en poblaciones no vacunadas. En países sin programas nacionales de vacunación, la incidencia general calculada es de 100 a 1,000 casos por cada 100,000 habitantes. Objetivo. Realizar una búsqueda exhaustiva mediante una revisión sistemática y actualizada de la Parotiditis, para obtener información reciente y estandarizada en el manejo de casos posibles, sospechosos y conirmados. Métodos. Se realizó una búsqueda de artículos originales, revisiones sistemáticas, y artículos de revisión bibliográica en bases de datos indexadas PUBMED, HINARI, SCOPUS, SCIELO, LILACS desde 2012 a 2018. Desarrollo y conclusión. Las parotiditis siguen siendo una enfermedad común en muchas áreas del mundo, su propagación depende de diversos factores asociados, pero controlables. Pese a que existe un programa nacional de inmunizaciones, la vacunación sigue siendo nuestra mejor defensa para la prevención y erradicación de esta patología reemergente. Es importante tener un conocimiento adecuado de esta enfermedad viral, para realizar un abordaje clínico certero, realizando un buen diagnóstico diferencial y brindando un tratamiento adecuado, para evitar las complicaciones, que, aunque sean poco frecuentes, tienen una alta probabilidad de mortalidad...(AU)

Resumen El virus de la parotiditis produce una infección benigna caracterizada por un aumento de volumen parotídeo que, antes de la introducción de la vacuna tres vírica, afectaba principalmente a niños y adolescentes. Luego de que esta vacuna se implementara en el Programa Nacional de Inmunizaciones, se produjo una notable disminución en su incidencia. Además, ocasionó un cambio en la edad y presentación clínica, siendo más frecuente en adultos jóvenes con mayor riesgo de complicaciones. Presentamos dos casos clínicos de parotiditis en adultos jóvenes confirmados por serología y en uno de ellos, por biología molecular. Se caracterizó el virus como del genotipo G, como el descrito en los brotes en E.U.A y Europa, diferente al virus contenido en la vacuna. El virus parotídeo sigue circulando en nuestro país y debemos mantenernos alerta ante eventuales brotes. Se hace relevante optimizar el diagnóstico etiológico por serología o técnicas de biología molecular con fines clínicos y epidemiológicos.


Mumps virus usually produces a benign infection characterized by increased parotid volume which, prior to vaccination, mainly affected children and adolescents. After the introduction of measles, mumps and rubella (MMR) vaccine, mumps incidence decreased dramatically. This intervention also produced a change in its clinical presentation, moving to young adult patients, with an increased risk of complications. We report two clinical mumps cases in young adults with different clinical presentations. In both cases, serologic assays were assessed and, in one case, a polymerase chain reaction (PCR) was performed in order to confirm the diagnosis. The isolated virus was characterized and identifed as G genotype, the same genotype observed during outbreaks in United States and Europe, and different to the vaccinal strain. Mumps virus is currently circulating in Chile and it is important to be aware of possible outbreaks. Viral diagnosis can be difficult, particularly in populations with high vaccination coverage. Therefore, the access to etiologic study through PCR and serology becomes more relevant in order to optimize clinical management and secondary prevention measures.

Resumen: Objetivo: Evaluar la prevalencia de anticuerpos antiparotiditis en niños y adolescentes de México, a dos años de haberse introducido la vacuna SRP. Material y métodos: Se estudiaron 2111 niños (1-9 años) y 2484 adolescentes (10-19 años). Se evaluaron anticuerpos IgG con un kit comercial de ELISA indirecto. Resultados: La seroprevalencia fue 70.6% (IC95% 69.3-71.9) y resultó mayor en adolescentes (83.0%, IC95% 81.5-84.5) que en niños (56.0%, IC95% 53.9-58.11) (OR 3.83; IC95% 3.34-4.39, p=0.0000000). Los niños de 1 a 2 y de 6 a 9 años, que a partir de 1998 formaron parte del grupo blanco de vacunación vs parotiditis, tuvieron mayor seroprevalencia que el grupo de 3 a 5 años no vacunado. Conclusiones: La seropositividad en niños de 1 a 2 y de 6 a 9 años fue probablemente atribuible a vacunación durante 1998-2000 y la de otros grupos etarios a exposición natural relacionada con el tiempo transcurrido en cada cohorte de nacimientos hasta el reclutamiento al estudio.


Abstract: Objective: To assess the prevalence of mumps antibodies in children and adolescents of Mexico, two years after the introduction of the mumps-containing vaccine MMR. Materials and methods: Evaluation of IgG antibodies with a commercial kit of indirect ELISA. Results: 2111 children (1-9 years) and 2484 adolescents (10-19 years) were studied. The overall antibody seroprevalence was 70.6% (95% CI 69.3-71.9), being higher in adolescents (83.0%, 95%CI 81.5-84.5) than in children (56.0%, 95%CI: 53.9-58.11) (OR 3.83, 95%CI 3.34-4.39, p=0.0000000). Children 1 to 2 and 6 to 9 years who were part of the target group of mumps vaccination since 1998, they had higher seroprevalence than the group of 3 to 5 years unvaccinated. Conclusions: Seropositivity in children aged 1 to 2 and 6 to 9 years was probably attributable to vaccination during 1998-2000 and in other age groups to natural exposure related to time elapsed in each birth cohort until the study recruitment.

Introducción: El presente estudio trata de un brote de parotiditis en un Liceo Militar que se inicia en junio del año 2016 y se prolonga hasta fines de octubre del mismo año. El objetivo fue describir las características epidemiológicas del brote. Métodos: Es observacional, retroprospectivo, de corte transverso de fichas clínicas de cadetes que constan en el archivo del Liceo Militar de Acosta Ñu que abarca el periodo junio a octubre del año 2016 y posterior entrevista a los afectados. Resultados: De 181 cadetes, todos masculinos, con edad media de 16 años, del Liceo Militar de Acosta Ñu. Se tomó como muestra a 115 cadetes (63%) presentaron parotiditis, sin afectación de las otras glándulas salivales. En 50 casos (44%) de forma bilateral y 64 casos (56%) unilateral. 23 cadetes (20%) presentaron complicaciones, como orquitis 22 (19%) todos unilaterales, pancreatitis 1. En ningún caso se presentó meningitis, encefalitis, miocarditis, todos sobrevivieron. De los cadetes afectados no fueron vacunados (SPR) 103 (90%), 8 (7%) recibieron una dosis y 2 (2.3%) dos dosis. Los casos ocurridos (63%) fue debida a la falta de vacunación completa (Triple Viral, 2 dosis) y a las condiciones de hacinamiento de los cadetes. Conclusión: El brote de parotiditis ocurrido en el Liceo Militar de Acosta Ñu, se caracterizó por la alta incidencia de contagios debido a la baja o nula cobertura vacunal y a las condiciones de hacinamiento de los cadetes. Por lo tanto recomendamos que al ingreso a toda institución que incluya residencia temporal o fija en condiciones de hacinamiento, la exigencia mínima debiera ser la presentación de un carnet de vacunación completo.


Introduction: This study describes an outbreak of mumps in a Military Academy that began in June 2016 and lasted until the end of October of the same year. Objective: To describe the epidemiological characteristics of the outbreak. Materials and Methods: This was an observational, retroprospective, cross-sectional review of the medical records of cadets at the Acosta Ñu Military Academy who presented for medical evaluation from June to October, 2016 and with subsequent interviews with identified patients with parotitis. Results: Of 181 cadets, all were male, with an average age of 16 years. We selected a sample of 115 cadets (63%) who presented parotiditis, without involvement of other salivary glands. In 50 cases (44%) the illness was bilateral and in 64 cases (56%), the illness was unilateral. 23 cadets (20%) presented complications, such as orchitis 22 (19%) all unilateral, and one patient had pancreatitis. No patient had meningitis, encephalitis, or myocarditis, and there were no deaths. Of the affected 103 (90%) cadets were not vaccinated with the measles-mumps-rubella (MMR) vaccine, 8 (7%) had received one dose and 2 (2.3%) received two doses. The cases that occurred (63%) were due to the lack of complete vaccination (MMR vaccine, 2 doses) and to the overcrowded conditions of the cadets. Conclusion: The Outbreak of parotitis, which occurred at Acosta Ñu Military Academy, was characterized by a high incidence of infections due to incomplete or no vaccination coverage and the overcrowded conditions of the cadets. Therefore, we recommend that upon admission to any institution that includes temporary or fixed residence in overcrowded conditions, the minimum requirement should be the presentation of a completed vaccination card.

PURPOSE: We report sharp rise of incidence of mumps meningitis at our institute and describe the clinical manifestations, laboratory findings, and complications. METHODS: This is a retrospective study involving 34 patients with mumps meningitis at Eulji University Hospital from July 2012 to June 2014. They were diagnosed by definite parotid gland swelling and leukocytosis in cerebrospinal fluid (CSF) study. RESULTS: Nineteen (55.9%) of the 34 patients with mumps meningitis were confirmed as positive by the mumps virus-specific immunoglobulin M (IgM) antibody in serum or CSF and the remaining 15 patients (44.1%) were classified as probable cases. We identified 31 boys and 3 girls clearly showing male predominance. The median age of the patients was 15.8 years and all patients were in the middle school and high school age group. Sixteen patients (47.1%) had unilateral mumps and 29 patients (85.3%) were diagnosed with meningitis after parotid swelling. Six patients (17.6%) were accompanied by testicular involvement and one (2.9%) showed hearing loss. The positivity of mumps virus specific IgM was 17 of 34 (50.0%) in serum and 4 of 34 (11.8%) in CSF. The total duration of illness including hospitalization was 5 to 21 days, median being 11 days. There was no serious neurologic deficit but 5 of 34 (14.7%) complained dizziness after discharge. It caused delay in back-to-work. CONCLUSION: It is worthy to notice that recent outbreak of mumps among adolescents are more complicated. We need to be more aggressive about prevention of mumps outbreak which bring on substantial socioeconomic loss.

To analyze the genetic characterization of epidemic mumps virus strains in Liaoning Province and provide the basis for mumps control. A total of 32 mumps viruses strains were isolated during 2008-2104. The fragment of SH genes and HN genes were amplified by RT-PCR, the PCR products were sequenced and analyzed. Basing on the 316 nucleotides of SH gene, The phylogenetic analyses were processed with the data of WHO mumps reference strains downloaded from GenBank and 32 mumps viruses strains. It showed that the 31 mumps virus strains belong to F genotype except MuVi/Liaoning. CHN/16.11 which was G genotype . Comparing to the A reference strains (Jeryl-Lynn and S-79), F genotype MuV were mutated on 12 amino acids sites and 27 amino acids siteson on HN gene. F genotype MuV added one N-glycosylation site in 464th-466th amino acids. The antigenic sites on HN were mutated on 121th, 123th, 279th, 287th, 336th, 356th and 442th. Maybe, it will influence the MuV antigenic.

In São Paulo the mumps virus (MuV) outbreaks have been increasing from In São Paulo the mumps virus (MuV) outbreaks have been increasing from 2011 to nowadays. MuV epidemiological surveillance has been improving by using the polymerase chain reaction in real time (rRT-PCR) in addition to the specific IgM antibody (IgM-Ab) detection; in some cases,genome sequencing studies were performed. Increased virus transmission and recent outbreakshave raised interest on MuV genotyping, as a means to understand the transmission pathways andto identify the vaccine-associated cases. From January 2011 to August 2016, MuV infection was analyzed at Institute Adolfo Lutz. A total of 232 (77.33 %) throat wash samples showed positivity to mumps genome, and 68 (22.66 %) were negative when analyzed by rRT-PCR. Among 15 samples for molecular analysis, 10 serum samples from respective patients were also available for detecting anti-MuV IgM-Ab; and from these, four (40%) samples were seropositive. Vaccination statuswas available only for patients from Cedral and Araraquara. Phylogenetic analysis revealed the circulation of the following mumps virus genotypes in the investigated periods: 2011(M), 2012, and 2013 (K); 2014 (N); 2015 (G, K, and N); 2016 (G). Knowledge on MuV molecular epidemiology in São Paulo-Brazil could contribute to the surveillance and epidemiological program in Brazil, and globally as well.


No estado de São Paulo têm ocorrido surtos de caxumba desde 2011. O diagnóstico laboratorialtem sido realizado no Instituto Adolfo Lutz utilizando-se a técnica de identificação de material genético viral por meio de reação de cadeia de polimerase-em tempo real (rRT-PCR) e peladetecção de anticorpos IgM (Ac-IgM) específicos circulantes. Os recentes surtos de caxumbatêm aumentado o interesse em investigar os genótipos dos vírus prevalentes para identificaros casos associadas à vacina. De janeiro de 2011 a agosto de 2016, 300 amostras de lavadosda orofaringe coletadas de pacientes suspeitos de infecção foram analisadas. O material genéticoviral específico foi detectado em 232 (77,33 %) amostras e 68 (22,66 %) foram negativas.Das 10 amostras analisadas pelo teste sorológico, quatro (40 %) demonstraram positividadepara Ac-IgM específicos anti-vírus da caxumba e seis foram negativas. Somente os municípios Cedral e Araraquara forneceram os dados referentes à vacinação. Análise filogenética mostroua circulação dos seguintes genótipos do vírus da caxumba no período investigado: 2011 (M),2012 e 2013 (K); 2014 (N); 2015 (GKN); 2016 (G). A vigilância virológica é mundialmente imprescindível, para identificar a diversidade e a distribuição dos diferentes genótipos, com vistasà composição de vacinas específicas.

In São Paulo the mumps virus (MuV) outbreaks have been increasing from 2011 to nowadays. MuV epidemiological surveillance has been improving by using the polymerase chain reaction in real time (rRT-PCR) in addition to the specific IgM antibody (IgM-Ab) detection; in some cases, genome sequencing studies were performed. Increased virus transmission and recent outbreaks have raised interest on MuV genotyping, as a means to understand the transmission pathways and to identify the vaccine-associated cases. From January 2011 to August 2016, MuV infection was analyzed at Institute Adolfo Lutz. A total of 232 (77.33 %) throat wash samples showed positivity to mumps genome, and 68 (22.66 %) were negative when analyzed by rRT-PCR. Among 15 samples for molecular analysis, 10 serum samples from respective patients were also available for detecting anti-MuV IgM-Ab; and from these, four (40%) samples were seropositive. Vaccination status was available only for patients from Cedral and Araraquara. Phylogenetic analysis revealed the circulation of the following mumps virus genotypes in the investigated periods: 2011(M), 2012, and 2013 (K); 2014 (N); 2015 (G, K, and N); 2016 (G). Routine vaccination against mumps has markedly reduced its incidence. However, the incidence of mumps continuously has increased since 2007. In 2013, a large mumps epidemic occurred in Korea, and this epidemic is still an ongoing problem. This epidemic occurred primarily in school settings and affected vaccinated adolescents, predominantly male students. The recent resurgence of mumps is caused by multiple factors: suboptimal effectiveness of the current mumps vaccines, use of the Rubini strain vaccine, waning immunity in the absence of natural boosting due to the marked reduction in the mumps incidence, genotype mismatch between the vaccine and circulating mumps virus strains, and environmental conditions that foster intense exposures. Containment of mumps outbreaks is challenging because the sensitivity of diagnostic tests is low among vaccinees and control measures are less efficient because of the inherent nature of the mumps virus. Despite the suboptimal vaccine effectiveness in outbreak settings, maintaining the high vaccine coverage is an important strategy to prevent mumps outbreaks, given that the routine use of mumps vaccines has substantially reduced the incidence of mumps and its complications as compared with that in the pre-vaccine era. In order to control the current mumps epidemic and prevent further outbreaks, we need to better understand the dynamics of mumps among vaccinated populations and the changing epidemiology in Korea. Concerted efforts should be made to systematically monitor the immunization status of the Korean population and to improve diagnosis efficiency. Furthermore, more effective mumps vaccines need to be developed in the future.

<p><b>OBJECTIVE</b>To investigate pathogens and molecular-epidemiology characteristics of viral meningoencephalitis in the monitoring sites of Zhejiang province, 2013.</p><p><b>METHODS</b>Cerebrospinal fluid and/or stool specimens were collected from suspected patients admitted to the monitoring hospitals in southern and northern Zhejiang province. Such specimen were subject to real-time qPCR for the detection of Human enterovirus (HEV), Japanese encephalitis virus (JEV), Mumps virus (MuV), Herpes simplex virus (HSV) and Cytomegalovirus (CMV). HEVs were isolated using the RD and Hep-2 cell lines, while VP1 genes from all HEV-positive isolates or RNA-positive specimen were amplified, sequenced, for homology and evolution analysis.</p><p><b>RESULTS</b>92 (38.5%) of the 239 samples collected from 229 patients were detected as virus nucleic acid positive, including 87 HEV positive samples, 1 MuV positive, 2 HSV positive, and 2 CMV positive; of the 87 HEV positive samples, 38 were further determined to be Coxsackievirus (CV) and 49 as Echovirus (E). 56 HEV strains were isolated from 239 (23.4%) samples. From the 31 cerebral fluid specimen of nucleic acid positive yet virus isolation negative, the most specimen were identified with E9 (9 specimen), followed by CVA9 (8 specimen); the viral serotype of Zhejiang province HEV were CVA9, CVB4, CVB5, E6, E7, E9, E11, E14, E16, E25 and E30, respectively. Predominant epidemic strains identified at southern and northern Zhejiang province were CVB5 and E6 respectively. The phylogenetic analysis of VP1 gene showed that all the HEV isolates in Zhejiang province were HEV-B.</p><p><b>CONCLUSION</b>The HEV-B was the main pathogen for viral meningoencephalitis in Zhejiang province in 2013, including 11 serotypes, while E7 was the first time to be isolated in Zhejiang province. The predominant isolates were CVB5 and E6 in southern and northern Zhejiang province respectively. The positive rate of viral nucleic acid detection was significantly higher than that of viral isolation. Regular EV isolation method was exposed to the risk of missing-detection of E9 and CVA9.</p>

The recent mumps epidemic in South Korea has generated a large amount of public concern. This study has attempted to analyze molecular epidemiological changes of mumps virus circulating in Gwangju metropolitan area, South Korea. 953 throat swab samples were collected from patients with parotitis from May 2013 to July 2014. The majority (71.5%) of these cases have occurred in middle or high school students aged from 15 to 19 years. All samples were tested using a reverse transcription polymerase chain reaction (RT-PCR) that targets the short hydrophobic (SH) gene of the virus. Mumps virus SH gene was detected in 39.2% (374/953) of samples. And 82 RT-PCR products were randomly selected for nucleotide sequencing analysis. All of these sequences were determined as genotype I by phylogenetic analysis and showed the highest nucleic acid similarity (99%) with Dg1062/Korea/98 (GenBank accession no. AY309060). These results suggested that appearance of new genotype or genetic variation at the nucleotide level could be ruled out to evaluate main cause of recent mumps outbreak in Gwangju metropolitan area.

RESUMO

We report a case of Mumps deafness with acute vestibular symptoms in a 13-year-old boy, who developed both parotid swelling preceded by acute right hearing loss and vertigo with spontaneous nystagmus. He was diagnosed as Mumps when the antibody of Mumps virus was detected in the serum. To our knowledge, this is the first case of Mumps infection, where parotitis was preceded by hearing loss and vertigo. This study indicates that the first symptom of Mumps virus infection could be hearing loss or vertigo.

BACKGROUND: Opsoclonus-myoclonus syndrome (OMS) is a rare neurological disorder that is characterized by involuntary eye movements and myoclonus. OMS exhibits various etiologies, including paraneoplastic, parainfectious, toxic-metabolic, and idiopathic causes. The exact immunopathogenesis and pathophysiology of OMS are uncertain. CASE REPORT: We report the case of a 19-year-old male who developed opsoclonus and myoclonus several days after a flu-like illness. Serological tests revealed acute mumps infection. The findings of cerebrospinal fluid examinations and brain magnetic resonance imaging were normal. During the early phase of the illness, he suffered from opsoclonus and myoclonus that was so severe as to cause acute renal failure due to rhabdomyolysis. After therapies including intravenous immunoglobulin, the patient gradually improved and had fully recovered 2 months later. CONCLUSIONS: This is the first report of OMS associated with mumps infection in Korea. Mumps infection should be considered in patients with OMS.

La parotiditis es una enfermedad infecciosa inmunoprevenible  causada por el virus de la parotiditis, miembro del género Rubulavirus, familia Paramyxoviridae, del cual se conocen 12 genotipos confirmados, designados como A-L y otro nuevo genotipo designado como M. Las vacunas anti-parotiditis por lo general,  se fabrican empleando virus vivo atenuado de alguno de estos genotipos y están  disponibles como monovalente (parotiditis) y trivalente (sarampión-rubéola-parotiditis). A pesar de los programas de vacunación implementados por muchos países, se han presentado brotes de parotiditis en forma epidémica en la cual se ha detectado co-circulación de genotipos entre poblaciones vacunadas. Entre las posibles explicaciones  están: el fracaso primario a la vacunación, pérdida de efectividad secundaria e infección por virus heterólogos. Como consecuencia la Organización Mundial de la Salud (OMS) ha recomendado estudios moleculares epidemiológicos, que incluya la genotipificación de cepas circulantes del virus de la parotiditis, como parte del programa de vigilancia. Esto permitirá una mayor información de la distribución de los genotipos en todo el mundo, contribuyendo a la vigilancia de la parotiditis y posiblemente en la reformulación de vacunas más eficaces. Esta revisión muestra la importancia que tiene la caracterización molecular  o genotipificación del virus de la parotiditis, con el propósito de comprender y explicar el comportamiento epidemiológico de esta enfermedad que ha sido ampliamente controlada por la aplicación sistemática de la vacuna a nivel mundial.


Mumps is a vaccine-preventable infectious disease, caused by mumps virus, member of Rubulavirus genus, Paramyxoviridae family, has been classified into 12 confirmed genotypes, designated as A-L and one proposed genotype, M. Usually the anti-mumps vaccines are manufactured using attenuated live virus genotypes and any of these are available as monovalent (mumps) and trivalent (measles-mumps-rubella). Although vaccination programs implemented by many countries, there have been outbreaks of mumps in epidemic form, in which has been detected co-circulation of genotypes among vaccinated populations. Possible explanations are: the primary vaccination failure, loss of high effectiveness and heterologous virus infection. Because of this, the World Health Organization (WHO) has recommended molecular epidemiological studies, including genotyping of circulating strains of mumps virus as part of the monitoring program. This information will allow greater distribution of genotypes worldwide, contributing to monitoring and possibly mumps reformulating more effective vaccines. This review shows the importance of molecular characterization and genotyping of mumps virus, in order to understand and explain the epidemiological behavior of the disease has been largely controlled by the systematic application of the vaccine worldwide.

<p><b>OBJECTIVE</b>To be acquainted with genetic characteristics and variation of mumps virus strains circulating in Hunan province.</p><p><b>METHODS</b>Mumps virus (MV) strains were isolated using Vero/ SLAM cells. The small hydrophobic protein (SH) genes of MV isolates were sequenced, and the sequences were analysed phylogenetically between the isolated strains and other reference mumps strains.</p><p><b>RESULTS</b>4 mumps virus strains were isolated from 16 specimens collected in 2011 from different regions of Hunan province. The genotype of isolated strains were supposed to be F type.</p><p><b>CONCLUSION</b>Genotype F is the main genotype of circulating strains in Hunan province in 2011 and there is no variation between genotype.</p>

<p><b>OBJECTIVE</b>To learn the characteristics of pathogen spectrum of Encephalitis /Meningitis in northwestern area of China.</p><p><b>METHODS</b>Between January 1st 2009 and March 31st 2011, a total of 569 patients with clinical symptoms of Encephalitis/Meningitis were selected from the hospitals in Gansu, Qinghai,Inner Mongolia and Xinjiang province. 1514 samples of specimen were collected from the 515 patients, to detect the IgM of Japanese encephalitis virus (JEV), enterovirus (EV, including Coxsackie virus, ECHO virus and enterovirus 71), Mumps virus, Herpes simplex virus (HSV) in blood and cerebrospinal fluid. Meanwhile, Neisseria meningitis (Nm), Haemophilus influenzae Type B (Hib), Staphylococcus, Streptococcus pneumonia, Streptococcus Suis, E. Coli and Cryptococci were also identified. The detection results were analyzed by different region, time and age range.</p><p><b>RESULTS</b>Pathogenic bacteria were identified in the specimen from 16 patients, with the rate at 3.65%, of which the dominant ones were Streptococcus pneumonia (7 patients, 43.75%). Virus were identified in the specimen from 132 patients, with the rate at 27.05%, of which the dominant types were EV and HSV, accounting for 33.33% (44 cases) and 31.82% (42 cases) respectively. The detection rate of virus showed a significant seasonal trend, with the peak appearing between June and November each year. The peak of EV detection was between July and September, with 24 cases detected out; the peak of HSV was between June and August (11 cases detected out); mumps virus was mainly found between July and December (25 cases). There was no significant time-distribution found in the detection of bacteria. The EV and HSV were mainly distributed in Gansu and Qinghai province (70 cases) ;most of mumps virus were found in Gansu province (24 cases);and JEV were only found in Gansu province (20 cases). The viral pathogen spectrum was identified in all ages, and the EV and mumps virus were mainly found in children aged 0-14 years old (42 and 17 cases respectively) ; JEV were identified in people over 15 years old, with 13 detected out of the 20 patients.</p><p><b>CONCLUSION</b>The main pathogen of acute encephalitis and meningitis in northwestern area of China was virus, and the main pathogens of encephalitis and meningitis in children under 15 years were Herpes simplex virus and Mumps virus.</p>
